What information do you need from me about the subcontractor I hired?

  • If you have not submitted your enrollment forms, then on Step 3 where you indicate amount of work self-performed, entering a value of less than 100% will cause the screen to expand revealing fields that, when completed, with give us the information needed to contact your subcontractor.
  • If you have already submitted your enrollment forms, please email your CIP Administrator the subcontractor's company name, contact name, email address, and phone number, then we will reach out to request their enrollment forms.
